Some schools may require your child to receive a physical every year OR every other year. This tends to vary by district. If you have been given one of the forms included in this paperwork by your child’s school, you may bring that form to their appointment to avoid filling out the same form twice.
While the front of this form may look similar to the New Patient Paperwork, massage therapy can address different needs or areas than chiropractic. Filling this out before your first massage therapy appointment helps our therapists get to the root of your discomfort.
